The last picot of the last inner ring is joined to the first picot of the first inner ring, when you get to that place in the tatting. It's just a normal join like all the other rings. You don't need to do any kind of fancy "folded join" there. The only time you need a folded join is when the tops of the rings in a motif are all facing outwards from the center. In this pattern, those inner rings are all facing inward - so the last join is not hard to get at to do.
