I'm confused by the gauge. It says 4.5 inches in stocking stitch but doesn't say how many stitches should comprise the 4.5 inches.

It is 4.5 sts per inch.

wondering if anyone has made this pillow using a different worsted weight yarn and how the guage worked out.

I've made a couple with Lion Brand Wool-Ease with great success! It's the same weight as the Caron Simply Soft that is called for in the pattern, so it's a good substitution. I also made one with Berroco Comfort that turned out great.

What a nice idea! You may want to consider sewing in a liner, the knitted fabric isn't all that sturdy.
