I used a perforating blade with my paper cutter to cut two lines of perforations: one 1/2" to the left and the other 1/2" to the right of the center of the paper, lengthwise. Then I cut the paper into (4) 8.5" by 2.75" sections, scored these on the center line, and folded the pages in two, to create 4.25" by 2.75" pages. I used an awl to punch two holes in the paper (1" from the top, and 1" from the bottom, respectively).
For the cover, I cut a piece of posterboard 9" by 3", scored it in the middle, and folded. I cut the decorative paper a little larger, to allow for 1/2" foldover on each side. I used a glue stick to adhere the decorative paper to the posterboard. I folded the corners, trimmed, them, and glued them neatly. For the inside of the cover, I used a piece of heavyweight ivory stationery, cut 1/4" smaller than the cover. I centered it on the inside of the cover, and glued it.
Assembly. I centered the book block on the cover, and used an awl to perforate the cover in the right places. Then I inserted a color-coordinated handmade cord (3 lengths of embroidery floss tightly twisted), tied it, and made a small bow.
